I can never give multiple duchy tier titles to a single individual, they go completely insane with power without fail.

Whenever I have a chance, I always give the weakest individual within the duchy the title. They're so busy dealing with their vassals they can't challenge me, most of the time.

To be honest nowadays playing the game normally doesn't work for me anymore.

What I'm doing right now is, after several runs of maximum oppression abolutism with direct royal control, creating massive sprawling empires that devolve into warring states.

The trick lies in making the emperor completely powerless, while giving his vassals incentives to fight one another.

I've found the best way to do this is to make every single duke and up in the empire of the imperial dynasty, so everyone has claims on nearly everything and everyone can call everyone into war.

Single Dynasty states are a toss up, they can be very stable or collapse constantly. A player can easily shatter them but that's the player. If the player acts nornally, the AI is a little stranger: often they'll actually be so busy forming into two giant coalitions to smash each other over minor claims they'll ignore the Emperor entirely. They'll also mass invade neighbours, especially if they're heathens.

A thing I have noticed that is interesting is the tendency for non-single dynasty states to steadily move towards that if the ruling dynasty has any significant holdings, your lesser kin will work together to slowly boot out everybody else.
