Do we know how the physical Sefar attacked Earth in the non-Extra timeline where there was no Moon Cell to 3D print her a body?
Probably, Velber 02 attacked directly, so instead of her ship crashing into the Moon Cell, it crashed on earth. Cue the world and its shenanigans with the holy sword.
As I see it, we don't even know if there is another reason why Excalibur was made in the no-Moon Cell worlds.
Moon Cell is "unique" in the EXTRA/EXTELLA worlds.
But, the thing is we also have Excalibur on the other worlds, while EXTELLA reveals that the sword was made because of EXTELLA shenanigans in the first place.
So, by simple logic, if Excalibur exists in worlds where there is no Moon Cell, does that mean that Velber attacked anyway? Is there any other reason why the world made Excalibur in those other worlds?
That's what we want to know.
We know that, in EXTELLA, Excalibur was made by the planet to defeat Velber 02 as no one and nothing else could manage that in the past, but we don't know if the creation of Excalibur has the same circumstances in the worlds where there is no Moon Cell. Maybe the world made the sword because it felt like it, or maybe there was another threat on par with Velber (IDK, maybe Velber 01 or 03?), or maybe it was just Velber 02, just that this time it landed on earth and destroyed almost everything without the Moon Cell "intercepting" it.
At this moment, that point is just speculation, but it's a lore reveal the fact that, at least in EXTELLA, Excalibur was made by the planet to defeat Velber 02.
Extella mats mentioned that in worlds where Moon Cell is not presented the circumstances of Altera would be different, but we also have random soldier in FGO Camelot chapter mentioning the titan. So it is most likely that it went straight to the Earth in no-Moon Cell worlds. In the first place the Moon Cell was only able to seal Titan Altera after the titan on Earth got blasted, sending the shock to the one on the Moon.
Oh, right. Forgot about that bit in Grand Order.
Maybe Kinoko will reveal in the future what those circumstances are because, I think it's pretty vague to just leave it like that. Did Altera owned most of the surface in those worlds, too?
BTW, I read your post about the possible teasing about the fifth in Link. Do you have screenshots or links with the videos? I'm really curious about that bit, but the streamer I watched some days ago got the videos deleted.

Nothing's happening with Aoko in Extra besides being forced to share a room with Touko for extended periods of time (which is really more of a mutual torment).
You could say it's significant that she appears there at all, but in that case I'll remind you that Shiki and Arcuied showed up there as well. And NPC versions of Kirei, Issei, Sakura...well, one of those DID end up plot important in CCC, at least.
