If Sakura had the worms removed from her, would she still be able to use magecraft? Would her original Element of Hollow return with time or would it still remain Water?What is the difference between having an Origin of Nothingness and Hollow?And was Shirou's Origin changed by the fire/Avalon or was it Sword even before the fire?
In HF she gets them removed and she can totally still use magecraft. She also uses her original element all the time, it's kind of a mix of it and Matou's magecraft.
Sakura personally removes the very last worm in HF and she still practices magecraft post HF. She never truly lost her own element either since big parts of what she is capable of as bad girl Sakura come from her original element, for example the ghost dudes and the warps.

I think Kotomine was actually talking about the main body there as he says that to remove what is left he would have to remove her heart, serving as a foreshadowing as to what Sakura does later. I mean, it's even directly said when Sakura removes it that the worm was hidden deep within her nerves and that was exactly the way Kotomine explained it too.She sticks her finger into her heart and pulls out a worm that was deep within her nerves.
It sounds as if they are trying to make the point that Zouken's worm was the thing Kotomine wasn't able to remove.

In regards to maximum Ea i only remember an instance of enkidu saying that Ea back in the old days is more powerful
It does make one think, why would Zouken even allow the operation to happen? It was also implied that it was because some of them were gone that she was even able fight off Zouken trying to eat her brain.
You could say it was Zouken's arrogance, but he didn't know Kirei had the spare seals or couldn't have just released Sakura since she was likely strong enough to survive the operation. So it was probably him knowing Kirei was crooked enough to not try to remove the main crest worm, as he has a good gauge of Kirei's mental illness.
