I remember Herc, Karna, Arjuna, Arash, Fionn, Cu and Sigurd being called that. I suppose Achilles too.

A couple times in FGO Chaldea detects 霊基 having a 大英雄 scale of mana. But I think it doesn't necessarily mean "Top Servant?"
"Top Servant" gets used a lot more on the Moon Cell side which makes me think it's a specific designation.

Pretty sure Alex was called a Top Servant by Nasu in 2015 in strict FGO context.
