The usual: Spelling, grammar, characterization. Then there's how it's written; I can't tell you how often I've tried reading something and just. Could. Not. Get into it for the life of me because I kept wanting to take it and give everything a once-over so it flowed more smoothly or just had fewer sentences that went "clunk." The tendency for writers to portray same-sex male relationships as strictly heteronormative rankles me a bit as well, though that doesn't come up nearly as often, least of all here.
If the only response you can think of for something is an overused terrible image with text on it or a beaten to death catchphrase, you should have a sit down and question your direction in life. "Who am I?" "Do I even have my own personality?" and "When did I replace my wit and social interaction skills with a series of shitty .jpg's?" are only a few examples of things you might consider pondering.
in fanfiction nothing really annoys me with the exception spelling errors and so on. I can deal with (and in some cases prefer) original characters. it's true it's nice to see a story about the people we know but too much of that is like the status quo in comic books. it's ok but doesn't give much room for growth. original characters can bring new perspectives to the table.
oh and people who don't actually read the work with an original character and still presume them sues based on what they've heard. it takes every step of a journey to justify the label mary sue/gary stu etcetera and i hate people who refuse to walk the mile and bitch anyway.

Dude, you're writing a fanfic, not the next great American novel.
This leads me to another thing, flowery, purple prose. Show, don't tell, but don't get TOO elaborate. Less is more, keep it simple stupid.
And bad porn. Stuff like not knowing anatomy etc. Also, it takes a girl a lot more to get wet than to just suck a guy off. She needs stimulation too, but that's a mistake I see all the time though.
