Both Hollow/Ataraxia and Fate CM2 state that the original Rider was Perseus, due to his association with the Pegasus myth. During the development of F/SN, however, they realised they lacked sufficient female characters, and decided to make Rider a woman instead. However, there were no female heroes associated with Pegasus, so they had to resort to using Medusa. (Rider's profile comments in Character Material)

I just dredged up CM2, you are right about it is mentioned in it that proto-Rider is Perseus (ペルセウス). It also talked about how proto-Rider's Master died soon after summoning, and he was battling solo (which is in-line with Character Materials).

Since it is from the horse's Nasu's mouth in CM2 (in a "creators' dialogue" format), it is probably the more reliable source. The "Theseus" in Character Materials may be a typo. We will see what the new tome says when it comes out.
