The Scarlet Devil of Gensokyo, Mistress of the Scarlet Devil Mansion.
Remilia Scarlet was born in the Outside World in 1502. Details of her past are shady at best, but it is known that she became the mistress and owner of the Scarlet Devil Mansion at some point between being turned into a vampire and coming to Gensokyo. Her sister, Flandre Scarlet, was born 5 years later than her. However, due to an unexplained incident after her transformation into a vampire, Remilia had her confined to the mansion during nearly 500 years. During this time period, she befriended the magician Patchouli Knowledge, who came to reside at the manor, and employed Hong Meiling and Izayoi Sakuya as staff, respectively as gatekeeper and gardener, and chief maid and personal attendant.
The Scarlet Devil Mansion first appeared in Gensokyo in 1998, though when exactly it was moved there is unknown. In 2003, however, Remilia decided to cover the whole of the Land of Illusion in a red mist that blocked out the sun so she could walk outside during the daylight. This became known as "The Scarlet Mist Incident" and marked the first recorded usage of the Spellcard System.

Magic Resistance: B - Cancels spells of a chant below three verses. Even if targeted by High Thaumaturgy or Great Rituals, it is difficult for Remilia to be affected. Having been alive for around 500 years and inhabited Gensokyo, where magecraft is at the level of the Age of the Gods, combined with her extremely powerful magical power, Remilia can brush off nearly every modern incarnation of magecraft.
Personal Skills
Regeneration: A - If provided with prana, Remilia can quickly regenerate and heal any wounds she receives, even if they be deadly or fatal wounds. However, the mana cost increases with the severity of the wound and relative speed at which she chooses to heal. This skill comes from Remilia's status as a vampire and her own magical potential, which grant her a nearly immortal body. In event of attacks that would almost entirely destroy her body, she can still regenerate by transforming into the form of a bat before restoring her true form. Nothing short of destruction of spiritual core can permamently kill her.
Magecraft: B - Remilia has knowledge of modern thaumaturgy, and is familiar with the intricacies upon which most magic theory is formed. Although she had the talent to do so, she has never invested herself in learning proper magecraft, and her magical abilities are mostly reliant on sheer power and its applications, among the which is her ability to produce powerful Danmaku and Spell Cards.

Remilia's weapon, a spear of blood and light powered by her magic. This Noble Phantasm is the reason why Remilia is qualifiable for the Lancer class. It has the characteristic of being unfailing when striking a target, no matter what the skill or strenght of the user. However, Remilia's Gungnir cannot be called the same weapon that was used by the Norse God Odin. To begin with, its shape is different from the original Noble Phantasm, because it is a weapon created by Remilia through her own ability. However, since its power is genuine, it cannot be called a fake either. It is best described as "the power of Gungnir given shape by Remilia's magic". And it is because of this that, while carrying the same proprieties as the original Noble Phantasm, its strenght can surpasses the original.

The true use of Gungnir by Remilia, the replication of the Spellcard by the same name. An ultimate attack which gathers all of Remilia's prana, converting it into light and energy, then throws it. The resulting effect resembles a high speed streak of red light that can pierce through most obstacles while tearing apart everything around the area it passes by means of the released kinetic energy. It can be made to instantly release all the accumulated energy in a large explosion once it reaches its intended target. If by some chance its initial strike is avoided when it has a designated target, the Gungnir will correct its course with sharp angles and return to its target.

The recreation of the Scarlet Mist Incident as a special bounded field. Once activated, it manifests as a thick red mist that blocks out the sun and makes the weather inside of its boundaries dark and cold. It can be extended to cover up to an entire city. Under the area of influence of this Noble Phantasm, the penalty applied to Remilia's parameters and skills while under the sunlight is rendered null, enabling her to use her full power even during the day. Apart from this effect, it does not have much power, seeing as its main function is remove the restrictions placed on Remilia during the day. While it can affect humans with an unconfortable sensation that is comparable to a morale-lowering effect, most magi can shrug off its effect simply by coursing prana through their circuits, and Servants are not affected at all. However, it does grant Remilia some limited awareness of anything that happens inside it.
After deployment, its origin point will be centered on Remilia, and will only vanish if she wills it, or is defeated, but additional restrictions may be placed for its release. If Remilia remains stationary in a general area, she can assign willing individuals to play the roles of the Gatekeeper and the Librarian. Doing this fulfills the conditions for the true reenactment of the "Scarlet Mist Incident". The effect forces any agressors to assume the role of the Incident Solver, which means that in order to resolve the Incident, they must proceed in the same way way as the Incident Solvers. The Gatekeeper, Librarian, and the Maid (Dog of the Devil) must be defeated before Remilia can be fought. The Noble Phantasm then disappears once she is defeated or acnowledges defeat and is sealed for the duration of the Holy Grail War.
Dog of the Devil - The Perfect and Elegant Maid
Type: Support
Rank: C
Summons the Perfect and Elegant Maid, Izayoi Sakuya, to Remilia's side. Rather than an ability that simply makes Sakuya appear whenever Remilia needs her, this Noble Phantasm is the crystalization of the bond between Sakuya and her mistress. It is the absolute loyalty that Sakuya has for Remilia which makes it so she will always come to her side. Since this is not a true summoning that abides by regular rules, Sakuya herself is only classified as a Heroic Spirit with general stats of D across the board. Even though she is outmatched by most other Heroic Spirits, she can at least hold them off thanks to a combination of her skills and her unique ability, Manipulation of Time. She always assumes the role of the Maid during the reenactment of the "Scarlet Mist Incident".
Chains of Fate - Manipulation of Fate
Type: Support
Rank: EX
The manifestation of Remilia's ability to "Manipulate Fate". It is a power that is always on, and cannot be turned off. The natural pinnacle of all mysteries that involve reversal and alteration of Fate. However, this Noble Phantasm cannot reach the full extent of its power because Remila can only use it subconsciously. How it functions exactly is not clear, but it will often bend Fate in order to produce an outcome that Remilia considers enjoyable or in a way that is advantageous to her. In addition, Remilia's mere presence can throw predetermined and probable outcomes into unexpected disarray and uncertainty of fulfilment. Merely shaking her hand can be enough to sway an individual's Fate in a way that will determine whether that person lives or dies.
It makes it so that any abilities or attacks with characteristics like "fated to land" are rendered useless against Remilia. However, Servants with a high enough LUK can resist or ignore the effects of manipulation of fate on them, due to already having the inherent gift to defy destiny.

Love it. Sakuya seems a little weak (most Heroic Spirits would die horribly to timehax, and besides she's fought Divine Spirits and won before), but that's easily fixed - keep the rank the same and say that since Remilia isn't a Grail-level entity she can't summon her at full power.
Spear the Gungnir is awesome, and poor CuChulainn will be so pissed at Manipulation of Fate .

Talent for Riding mounts and vehicles. Under normal circumstances Caligula gains the knowledge to effortlessly ride any earthly mount or vehicle, yet with use of his Noble Phantasm Neos Helios this Rank can be raised even higher to the point where the likes of Divine and Demonic Beasts are within his grasp.

Charisma (Rank A > E)
A rare skill; the talent to lead an army and inspire a nation, having the effect of increasing the abilities of allies in group battles.
Caligula can be considered an... unorthodox example of this. Upon first being summoned his Rank is befitting of those who've attained the highest imaginable level of popularity, yet as the Grail War goes on this slowly yet surely (or perhaps not slowly at all) descends to the absolute lowest, where though he may efficiently lead his followers he will not work wonders for their morale, to put it lightly.
Divinity (Rank A--)
The measure of divine blood inside a demigod. A very distant descendant of the goddess Venus, via his ancestor Aeneas, and a slightly-less-yet-still-fairly-distant descendant of either Mars or Heracles, via Romulus. However, unlike previous Roman Emperors Caligula received deification while still mortal, as opposed to merely after death.
Can be boosted to new extremes through the use of, again, his Noble Phantasm Neos Helios.
Innocent Monster (Rank C)
The name of the monster who had his past and way of being distorted through te image spawned from his behaviour in life. Incidentally this Skill cannot be unequipped.
Though there is enough evidence of Caligula's madness, it should be noted that his reign is considered to be poorly documented with definite bias against him, so it's quite likely we haven't been getting the full story on him.
Mental Pollution (Rank E > A)
Capacity to block out Mental Interference due to possessing a distorted mentality, yet at the same time it becomes nigh impossible to form an understanding with others unless they possess an equivalent Rank.
As a mirror to his Charisma Rank, this gradually increases the longer the Grail War goes on.
Noble Phantasms
~ Baiae - Every Chance of an Emperor (Rank C, Anti-Personnel) ~
Tiberius' soothsayer Thrasyllus of Mendes once prophesised that Caligula had no more chance of becoming an Emperor than of riding a horse across the bay of Baiae.
In response to this, after he did become emperor Caligula ordered the construction of a 'floating bridge' made possible through numerous ships together in a line, over which he did cross the bay riding his favourite horse Incitatus, wearing Alexander the Great's breastplate no less.
A crystalisation of this legendary stunt (and perhaps also the reversal of Caligula's own popularity), whenever mounted upon his ever-ready steed Incitatus Caligula receives the attribute 'Anti-Expectation', allowing him to intentionally defy any expectations of him his enemy may hold, in turn allowing him to pull off nigh impossible feats while riding.
Incitatus' own un-horselike qualities such as speech and religious knowledge may possibly be a side-effect of this Noble Phantasm.
However, this Phantasm may sometimes be more of a curse than a blessing for Caligula, for example if he faces an enemy with not lower but higher expectations of him (unlikely due to his sickly frame and delusional personality, but still) this NP can instead end up restricting his abilities and even afflicting him with misfortune.
This means that Baiae can only effectively be used against each enemy once, unless they're that arrogant and assume that last time was but a fluke.
The only way not to trigger this Phantasm at all, for good or ill, is to simply face Caligula without holding any expectations.
~ Speculum Dianae - The Lunar Seconda Nave (Rank B, Anti-Fortress) ~
At the otherwise restricted holy ground of Lake Nemi Caligula ordered the building of two massive ships, both of which have been ranked as some of the largest built in the ancient world.
This Noble Phnatasm is the smaller of the two ships, designed as a temple to Diana or Isis, or possibly a synchronisation of both, though nowadays Caligula tends to look at it as a temple to his sister Drusilla.
The ship Speculum Dianae has the power to absorb faith and belief, or more accurately drain them away, sucking up any prayers or devotion it comes across and taking them into itself.
This can lead to any ability dependent on prayer or a connection with the gods being disabled and stolen away if used in its presence, can induce loss of hope and lack of willpower in anyone religious yet not mentally resistant enough, and after prolonged can even cause the destruction of any holy site, starting it by causing the heads of any religious figures to fall off (this being a direct nod to Caligula's own attacks against temples).
'Speculum Dianae' (reportedly translating to Diana's mirror), comes from the name given to the enchanting reflection of the Moon that may be glanced in Summer upon the waters of Lake Nemi, or Nemorensis Lacus as the Romans knew it. If you go by Tarot symbolism, the Moon itself may be a fitting allegory of Caligula's own madness.
~ Neos Helios - The Solar Prima Nave (Rank A, Support) ~
This was the larger of the two ships built at Lake Nemi, a massive floating palace of the highest luxury and grandest envisioning, probably the most extravagant yet outlandish of Caligula's constructions.
As a vessel it is a good deal sturdier than the Seconda Nave yet also far less agile, however its true function is not of a battleship but rather a temple, to none other than Caligula's own Divinity itself. The title 'Neos Helios' stems from Caligula's personal name as a God meaning the New Sun.
Within this floating palace he is able to take the 'faith' accumulated by his previous Noble Phantasm and transfer it into his own self, allowing him to imitate the likeness and powers of any God (or Goddess) who had any belief in them drained away by Speculum Dianae, with Incitatus in turn being granted the capabilities of any priest assigned to that deity.
Alternatively, Caligula can draw upon all the accumulated belief at once and pile it into himself as God, if things are looking desperate or he just wants to be done with his foe.
Neos Helios is the closest he can get to realising a world where everything his madness led him to believe is absolutely true. This last Noble Phantasm is akin to a Reality Marble in that while it grants access to powers far above human, even Heroic Spirit level, Caligula can only use these miraculous powers inside this ship, with no effect or result of these powers able to be carried to the outside. Therefore, in order to vanquish his foes they must first be lured into this ship, which may be accomplished easier given the destruction Speculum Dianae can cause.
Analysis
Though his Noble Phantasms are capable of major results if used well, without them Caligula ranks as a comparatively poor Servant, and with or without them he remains an extremely difficult to control one for any Master unfortunate enough to be saddled (pun possibly intended) with him.
Summoned along with him due to their close ties, Incitatus too can be viewed as a Heroic Spirit of sorts much like Bucephalus, with a Rank in Independent Action of around D, letting him assist Caligula mounted or unomunted (if that's the right word).
Incitatus' personality seems to be much more level-headed than that of Caligula's, though inevitably he finds himself drawn into whatever craziness Caligula may be plotting.
While Caligula may speak of countless (and often contradictory) desires he has for the Grail, such as abolishing all other religions so that only he is worshipped or being allowed to survive his assassination in history and continue his reign where he left it, deep down his one true wish for the Holy Grail would be simply to be loved again, like the adoration he received when he first ascended the Imperial Throne, the one moment in his adult life he remembers the fondest.
This is probably the one Servant profile I personally feel I've put the most effort and thought into so far, hence why I wanted to display it. Though I'm a bit worried about potential OPness, at the same time I'm not too fussed, due to the fun I had thinking up the NPs.

I will never get tired of seeing people like you come up with such original and well structured ideas based on the feats of historical characters. The machanics of his Noble Phantasms and their justifications make him one of my favorite servant profiles I've ever seen.

The mark signifies Cain’s status as a traitor, revealing his identity instantly to whoever sets eyes on the mark. However, the mark also signifies God’s punishment to Cain: that he is to be a restless wanderer and that none should seek to end Cain’s suffering through death. Thus, whoever sees the mark will know of his identity but will not be capable of inflicting harm upon him. The simplest way to get past the mark’s effects is to close one’s eyes and fight with the other sense. This is why Cain is of the Assassin class, as the opponent is forced to fight someone he “can not see”.

For a first try in a Servant profile, not too bad mate. Though, sorry to say, but there was another Cain profile around that totally blew this one out of the water.
Not sure if it counts as first try actually...I've been doing this stuff since I finished all three routes... Though I mainly use historical and mythical figures instead of modern media, but that's just me.
Said so only because your account, if nothing else, is new.
I originally wanted to post Berserker Draupadi or Lancer Kaguya-hime next (tight fit, but they could apply for those Classes), but upon hearing the interest in confirmed historical figures I decided to go with this guy. Maybe a bit too obvious a choice (though I'm surprised I haven't seen a profile for him anywhere else on site) and I guess not as fun as Caligula, but anyway:

However, Phaeton was not skilled enough to fully control the horses of the sun and lost control upon his descent. The chariot is Phaeton’s greatest weapon but comes at the cost of destroying him when he tries to dismount or after half a day.

Pretty cool profile! Richard the Lionheart, eh? He's a figure we usually see much exaggerated in children's stories. By which I mean, he's depicted as much more lawful and good individual. Interesting use of demonic heritage in his family backstory to add to his repertoire of NPs. Good sir, you write some nice Servant profiles. Any chance we could see Kaguya-hime next?
