I don't get why Kotomine would die in the first place after unleashing AM.
He has a contract with AM, and AM looks out for him. He can manipulate the mud. Unlike most people he should be able to survive, and Gil acted like there would indeed be survivors.

He doesn't; he just got the cursed heart as a side-effect of Gilgamesh telling All Evils they sucked.
The stuff he does at the end of Fate is just a result of that; the actual Angra Mainyu would still kill him since he's human.
