I know DMPC's are normally frowned upon, but mine always seem to have the worst luck.
My poor magus got crit like a truck while at half HP and flat-out died. I didn't actually mind too much, as he mostly just existed to test out my extensive list of houserules and wasnt as fun personality wise as some of my others, but still seems like an odd tendancy.
I seem to be encountering the unfortunate rut where I, the DM, am bored with the setting and story, but my players all seem to be enjoying it.

I've caused 1 TPK myself before as a player when I dropped a stronghold on their heads filled with specters and a dragon. It was fun. My DM just TPK the whole party with a single banshee I got away but he got me with a Chimera. And I almost TPKd my group in a sword fight and some well placed backstabes put the party at my mercy.
tl:dr The point is when you TPK them make it fun for you.
Why are you bored with the setting and story?
He never sleeps. He never dies.
Battle doesn't need a purpose; the battle is its own purpose. You don't ask why a plague spreads or a field burns. Don't ask why I fight.
