Now that 1000 years have passed, even the Umbral Star events have been ignored, so I don't think Sakura will be that important either in this Extra "alternative" (not that she really was in the first place anyways beside giving you lunch boxes).

Also, now that SE.RA.PH basically non-existent, is there even a Regalia? Is Altera still there?

It exists, but thanks to "Chakravartin", it's ineffective and can't really enforce any rules or actually control anything that happens inside of it.
That's why I'm surprised about this 1000 years skip. If SE.RA.PH can't enforce its rules, the HGW can't conclude and the Moon Cell can't hand the Regalia to any Master.
And if the Regalia isn't in the possession of anyone, how can Twice fight Velber?

Maybe Velber is the reason Earth is in the state it's in? Unless we've already been given a reason. I haven't watched the episode yet.
Mostly it's just because that's where EXTRA was always headed anyway and if the Velber were involved there would be a lot more giant ladies stomping around
In Extra humanity was already down to 2/3 of the population, I'm more surprised that they held on another 1000 years after that.
If Velber went to Earth I don't think there would be anyone left, maybe Twice would have made a miracle happen the first time if Altera came but I doubt he would be able to defeat the main body of the Umbral Star which came just after :/ I think humanity is in such a poor state, Velber just doesn't need to act. Humanity will destroy itself...
I think it only attacked because Hakuno and Nero as the winners of the HGW gave hope and a future to people.

Well, Velber appears every 14,000 years, so there are no ifs about it.
So, unless the LE world doesn't have the Velber in the first place, or someone faced it and either:
1) Won, without the help of the Moon Cell (Counter Force?). In this universe, the Moon Cell must've moved far away from its position because it'd be targeted by Velber as well, with no one to protect it, unlike Earth.
2) Won, with the help of the Moon Cell, which means that the Moon Cell acknowledged a winner (maybe Twice?), and gave him the Regalia to defeat Velber. If that 's the case, what even is happening in LE?
Or 3) Conspiracy theory time: Something something time is different inside the Moon Cell, which kept rewinding Hakuno (thinking he's an NPC) to help defeat Twice, get the HGW underway to get the Regalia in the hands of someone who can face Velber something something.

If it's her state seen in Extella yes, but with her skill "Crest of the wandering star", if she reached her final stage, she would have almost the power of the sun and that would be an instant dead end for this timeline.
