Mmm, Xenogears for me as well, I think. Actually a ton of Squaresoft intros are quite memorable - FFs, Chrono Cross, KH.
I liked the start of ME3 quite a lot also.

Well, namedrops are intentional and skin-deep for the most part.
Xenoblade has a really cool intro and I can very vividly remember ff4 and 6's intros but only because I've tried to play those games many, many times.
The best game intro sequence to me, movie-wise (that I can think of anyway) is probably the one in Soul Reaver 1: https://www.youtube.com/watch?v=zXZV5QMDftU (make sure you switch the quality to HD). As for intro sequences in general, with movies+in-game cutscenes+possibly gameplay, I don't know...maybe AC2 like someone mentioned.
