I just found out that the one who translated this is chapter is a guy I know from psylocke. Also the one who introduced me to my raw provider. I feel a bit bad now.

Dude I never said all adaptations are worse than the original, and never would i dare to utter such nonsense.
All I said is that it takes way more than just Apocrypha being average (in your opinion) and the author to be good to have a good adaptation. (good in the sense better than average since you were talking about it being better than the LN)
You never know when those things come in handy...

That exactly how it sounded (putting aside that Ad hominem remark for now). Anyway Ishida already used decent LN (WN) and made it better and Apocrypha has LOT more room for improvement than Kotowaru.
Could have just been in too much of a rush. Misreading words by replacing a letter/character in the middle of a word is not all that uncommon even in one's native language.
That said, く doesn't really look a whole lot like う... They probably just skipped over the character entirely.
They're probably a relative novice. (Mis)read the furigana too quickly, saw a word they knew, and didn't bother double-checking the kanji. Reason I say that is it sounds like the kind of mistake I would make, though I would hope that if it were me I'd have taken a second look, since I'm pretty sure Fuyuki isn't near Kyoto...
極東 =/= 京都
They are not related at all. The only way to make this mistake is if you couldn't read it at all, looked it up, saw the pronunciation, and then mixed up the pronunciation (but not the actual word) with something else.
