As for the manga itself, what it lacks in backgrounds it make up for it in some particularly beautiful Character art. This is honestly the first time I've ever considered viewing D'Eon as 'he'
It's one of the difficulties I see with viewing FGO in a different medium since the game reuses enemy mob assets regularly and leaving it alone eventually makes it an obvious gameplay element of a video game that detracts from the attempt of adapting the story in the first place.

Nope, it wasn't among the ones listed. While it's technically an EOR chapter, it comes in the form of a collab event and unlike the others can actually be missed if you weren't around for the collab so it's not being adapted
Whether it is an intentional artstyle or lack of ability doesn't really change how it looks.
Ishida Akira draws similarly wide faces, and he's clearly a professional artist with skill. That doesn't make them any more pleasant to look at.
