I don't see what's confusing. I think Mereo made it pretty clear it was Sakura's viewpoint. She feels violated and unworthy, something she goes on about in HF in detail. Since Shirou doesn't know about it, he has no way to comfort her so her misery festers. And in the end, she is only a substitute for Saber in Fate. And for her, that's still enough because she has zero self esteem.
