You're dwelling too much on physical pain, I think. The one thing that struck me about his deaths in canon was how quick they were--he barely saw Berserker before he went splat in Fate, and he never even realized he was dead when Sakura offed him in Heaven's Feel.

For his death in this fic, I was more interested in emotional pain. I wanted him to see it coming (well, in a matter of speaking) have a chance to dwell on it for a moment, and completely break down before his life ended.

Not really. I thought it would be nice if Shinji gain the ability of using circuits to draw prana (like a proper magus) just as he wanted, only for him to lose control. As the circuit starts to burn his body inside out, Shinji starts to realize that it was his wish that caused it. I think it would cause more impact on Shinji that way. The high physical pain is just an added bonus.

Shirou has bigger concerns than his lack of a harem.

Not really, it's that what Sleepy is describing is a probably very interesting but completely different take on a plot point that happened aeons ago.
So much that I'd argue it would make more sense as a separatepost in the FF ideas thread.

He can always have a relationship with someone else and considering the tone of the story lots of characters are going to die as well, so I'm sure he'll have someone lined up.
