I don't know about that. From what you see in game and subsequent material, that "depedence" seems to be something that they are able to get over, and the events of the game were just one set of really terrible events. It all depends on whether you think they are capable of change or not.
Let's look at their daily lives. Even if Sakura gained interest in cooking and archery because of Shirou, she pursues those things outside of him. She isn't really the character that just learned cooking for her crush, she actively enjoys it and tries to get better outside of working with him. With archery, she stays the course on it and leads the club, sure, she asks Shirou to come back once or twice, but all things concerned, Ayako seems to be more into bringing him back on.
She also doesn't visit Shirou every day, much as I'm sure she would love to and has the right to some peace. Add to the fact that she splits the food bill with him as well. They seem like pretty familiar tight knit friends and an almost family like atmosphere with Taiga.
On Shirou's side, he speaks to her in the most casual way, since even when he gets to know Rin and Saber, he seems to revere them more both internally and outside his head (beats of snark notwithstanding), and just enjoys himself without much fanfare of "it's a girl, whaa". Though we could argue whether that is a good or bad thing too.
So I say I can see them just being a very laid back type of couple, even if they had issues, they seem capable of getting through it with communication.
Basically, if you never knew how fucked up they were on the inside, and viewed them from afar when interacting together, they are pretty normal.

I don't think it's said that they'll die. Shirou can most likely get tune ups without sex, it's just hey, if you two are going out, why not do it this way and have it be enjoyable? And they do mention trying to find alternatives.
On Sakura's side, while magical energy constantly collects in her body and she needs to let it out periodically, she isn't sweating bullets or having a stopwatch saying, "Nee-san, we have ten minutes to end this reunion because senpai and I have to bang or he and I will explode."
So like the end says, they'll have to work out the side effects their bodies have to mantain a normal life, but they seem capable of doing that and plan to make preparations to work it out.

Like, Sakura knows that Shirou has a fondness for Rin, because feelings don't necessarily go away just because you've picked a partner, and Rin is still much more of a capable magus than she is, so she could fulfill the role of tuning his body.
Even then, Sakura doesn't seem worried about that in the slightest, even makes fun of the awkwardness that they show toward each other, not even in a passive-aggressive way.
I mean, if you want to go even further and say Medusa being around is the metaphorical gun to his head, then okay, but I kinda doubt it's anywhere as sinister as you're making it.
