That's what I got from it. That one of the lines happened to go over the chest, and that's why it was a guaranteed instant kill. Because there's, like, important organs and stuff all up in there.
What's to stop Shirou from just Tsubame Gaeshi-ing everyone? Summoning the sword is easy enough for him and since the cost to deploy it is minimal then it should be easy enough, after all we know Nine Lives is possible.

Actually, i dont remember all of the Fate-route, but did Shirou ever actually see Tsubame Gaeshi? Maybe he just doesnt know about it.

We know he at least sees it during the Fate and UBW routes, maybe even the HF route if you count that time when he sees the katana flying away when Assassin dies.
As for actually seeing it we also know he wouldn't have to as he did the exact same thing with Nine Lives.
While i hadnt thought of it before, it is true that shirou can use the memories of the traced swords to fight like their actual users (which he does in Fate-route with Caliburn, probably in UBW at the end, and with Herc's sword to use Nine Lives), Nine Lives shows that he can do it even if he shouldnt even physically be able to, so i guess him tracing the laundry-drieng pole and tsubame gaeshing at least once doesnt sound impossible.
Yes, Nine Lives are possible, and it requies him to be in a very dangerous state. So does Crane Wing. So does tracing and invoking Excalibur.
HF Shirou never saw Tsubame Gaeshi, but maybe then he could do it.
At least Archer should be able to do it if Nine Lives, which aren't just tied to the weapon itself, are possible.
Even then it's not like he would Tsubamegaeshi everyone. Reading a technique like that is pretty serious bussiness and in the few instances we see him do it, he fries himself(Crane Wing), dies(Excalibur), or it's not enough anyway(Nine Lives).

@Polly you are aware that Shirou traces the sword's memories (aka the techniques the user of it did/can do with it) with the sword, are you? Thats how 9 Lives Blade works worked.

Yes, I remembered that eventually.
But with Kojiro being a fake and everything, it wouldn't surprise me that his sword doesn't hold the "memories" of TG.
Not to mention Shirou can't get Caliburn's "7-lives-kill" effect because he's a faker and it takes Saber's help to activate it, and Nine-Lives might be attributed to Archer's hax arm, so I'm a bit skeptical to how well Shirou can trace the memories of a technique.

He doesn't actually see Assassin do the technique in Fate or UBW, just briefly glimpses Saber staring the guy down, so he'd have to realize there's a technique there to begin with, like Berserker's Nine Lives.
It would have the wraith's memories, since it's pointed out that fake Kojirou's 150 cm sword probably isn't actually the 90 cm one if-he-exists-real-Kojirou would havethere's a penis joke here I dunno I've been off my game today...
Localizationing stuff
The arm was assisting in projection, but its still something he could do on his own, same as seeing Archer project and then later use UBW in the UBW route helped him gain better skills. Besides, Herc's sword is ultimately a big piece of rock in the shape of a sword, Kojiro's weapon should count, whether he is a fake or not.
