Berserker Lancelot isn't.
He's mentioned to be like a vengeful ghost, like how Medusa was described or something along those lines.

This black knight definitely didn't have any of the "radiance" that bore the other Servants who were already there. Arturia, Diarmuid, and the King of Conquerors Alexander as well as the yet unannounced golden Archer all have that "shine". This is the expression of the pride of a Heroic Spirit. The honor of the legends everyone praises and longs for. This is an essential element of their "noble phantasm".
But the newly appeared black knight had none of it. You could say he was closer to an Assassin. The darkness around the black armor definitely held a "negative surge".
Thus, more than a Heroic Spirit, he might be labelled a vengeful spirit instead...

So if you are a legit hero but are not heroic as a berserker, as in, you are moved by other emotions besides primal rage, you need Avenger to play overseer? What?
Before Avenger, the Fuyuki Grail only summons proper 'heroes' (Hassan aside). It doesn't have anything to do with Berserker in particuliar.
People who were driven into insanity and became not-heroes like Gilles or Lancelot aren't proper heroes anymore and wouldn't be summoned.

Now it'd be interesting to know what exactly makes them not-heroes as Berserker when they could normaly be summoned as other classes.
Also whether this also works to "exclude" other classes for certain heroes
