I can't speak for the Nasuverse, but depending on the source you read, there's only one Lady of the Lake, who seduces and imprisons Merlin, raises Lancelot, gives Excalibur to Arthur and is then beheaded by Sir Balin. If I'm not mistaken, Malory separates the above in three different characters, all called "the Lady of the Lake" at different points: the one who seduces and imprisons Merlin (Nyneve, I think), the one who raises Lancelot (Vivian, if I'm not mistaken), and the one who does all the other stuff (...I'm not sure she's even named by Malory, actually, and I can't check right now).
I don't know; considering it's the crystallisation of Herakles defeating the Hydra by slaying all of its heads at once, why wouldn't the original Nine Lives be 9 simultaneous attacks?
Nine Lives (Sword) isn't the recreation of a legend, it's just Herakles using a sword technique based off of the Nine Lives Bow, which was a magical bow.
