Not to mention that Nine Lives kind of seems like overkill for your run-of-the-mill beasts, possessed corpses, and broken down homunculi. Especially considering that every movement was causing Illya extreme pain, you'd think they'd value conservation of energy.

Don't know, there's not a single mentioning of Mad Enhancement in the entry. Just two notes saying while summoned as a Berserker, Hercules is completely locked out of Nine Lives. Once in the title, as listed above, another in the text.

I'm not sure, is it explicit, or just another "can't use as Berserker"? If it is the latter, then it changes nothing, since Berserkers are supposed to be Mad Enhanced all the time now.
Also, does this article says that he can't use his Eye of Mind?

It looks like it is actually the Class that locks Berserker out of Nine Lives, which makes sense, because Nine Lives is not listed in the status screen at all.
Also, yes, Eye of the Mind entry explicitly mentions Mad Enhancement.

The Nine Lives entry seems to repeatedly and deliberately emphasize it is the Class, not Mad Enhancement alone.

You can take the literal interpretation, in which case you'll have certain problems, or you can take the an actual reasonable view, in which case it avoids certain problems.
Admittedly there's some cost since it's not explicit and without ambiguity, but that's still better than the alternative.
