Hmm, an idea (I seem to have lots of them lately).
Ryuunosuke sacrificed family with magical potential to summon caster, using their blood to perform the summoning, but leaving the last member of the family alive.
So, what if he wasn't considered the master of the war? What if that kid was? After all, it is his family's lifeblood that was used.
Caster would likely be representing "hope" or "being saved from the brink of death" or something along those lines. Or it could be EMIYA on the basis of connection through being "victim of the Fourth War" as well as coming from family with magical potential living in Fuyuki (or something like that).
It would obviously change the war, but I'm unsure how it could play out.

I think that people generally recognise that they're hurt when their body has a massive gash in it, even if they're not in pain....

Shinji is desperate to learn magic right? So what if he finds a book on demon summoning, and makes a contract with an old one to be able to cast magic. Let's say his price is he has to sacrifice a certain quota of people to his new God.

Shiki would do SO WELL in Fate. His philosophy is the opposite of everyone's there, barring Gilgamesh's and maybe Kotomine's. Am I assuming that Assassin Satjuski would be involved?

There is a certain room in a certain place that grant wishes. It will give you the thing that you crave for. The desire that has made you suffer the most but be wary.
The room is not a genie in the bottle. It will read your past and drag out your deepest most precious desire no matter how large or small.
Of course it is no easy task getting to this room. You need to be guided by a ferryman. A man who has found this room hundreds of times and guided many to it for a small fee of course.
The ferry man most never enter the room himself, or he will be punished.
In Fuyuki there exist one such ferryman who will help you realize your deepest ambition, Kotomine Kerei.
Not certain with this premise, it mostly would probably be short stories of Kotomine trolling others.
