and nasu does have specific sci-fi shit. that is why we have Extra. some stuff occasionally ties into that, but that is it

The sweet spot stakes-wise is something that's really important for the characters, but in a relatable way, and enables character conflict.
Go too high, and it becomes unrelatable. Go too low, and it becomes trivial.
Another problem of super high stakes is that their very nature tends to involve tons of characters, so there's a real risk of diluting focus. It's harder to make a story with super high stakes and a reasonably, well-knitted / developed cast (for what little I know about it, FGO doesn't even try)
High stakes are better when you slowly start building up from something small. Fate route is a good example - only very late in the story it is revealed that world destruction is at stake, and by that time we already know the characters and their motivations well enough.

The Holy Grail War was a failed ritual in a magical backwater. The major conflicts of Tsukihime and Kara no Kyoukai revolved around some serial killers, however supernaturally charged they may have been.
I don't give a rip about Nasu's time-traveling, dimension-hopping, humanity-erasing, husbando-and-waifu-collecting fuck-a-thon.
Give me intimate stories about damaged people or give me death.

"intimate stories about damaged people" is a very broad category with a lot of potential for new stories. It won't get repetitive any soon.
But back at the topic of stakes, one aspect no one mentioned yet is how tension happens when the hero side's loss is plausible. If the stakes are the literal end of the world, a loss is never an option, so a reader will never believe it can happen and tension drops to zero. Of course, this kind of tension is not mandatory to any story, you can still make a lot of great stuff without it, but it's always preferable to have.
The stakes nasu stories are never about winning or losing, they are about what needs to sacrificed 9n the way to victory.
