That could just mean that the Tohsakas got wiped out in the fourth war.
Third family would be Miyu's, being the ones to supply the grail vessel and all.
Maybe the Tohsaka never existed, Matou moved to Fuyuki and became second owners, and then the Sakatsuki settled there later when they started working with the other two families to create the grail.
Then who was Sakura
Wasn't the whole point of Shinji being harsh on Sakura because she was an outsider who was more talented than him? Would Shinji still be such an ass if Sakura was his blood related sister?
It's not so hard when you realise where are diverge points from main FSN timeline. For Prismaverse it's sometime around Fourth Grail War, which explain, why that world is mostly same and just outcome is drastically different. But for Miyuverse diverge point started already during time of creation of Grail War (which is most distant diverge point out of all Fate spin-offs so far), making Miyuverse basically completely different world just with same rules and similar features. Then you must ask: What is different? What is same? And what we know about it?
Regarding Three Founding Families, there are absolutely no traces about Einzberns so far. Which means that they either never existed, or never left Germany. In any case it means that their aspects of Third Magic were never integrated into Grail War ritual, which explains why there are no materialised servants.
Role of Makiris/Matous seems mostly same so far, which explains why technical aspects of Grail War are still same.
About Tohsakas we currently know moreless nothing, but Grail War still occur in Fuyuki and Sakura exists, first suggest that they were involved in creation of Grail War after all, and second confirms that they had to exist at some point of time, but now they are apparently gone.
Ainsworths and from what we know about this family, their magic is displacement magecraft which can overwrite the laws of world, which makes clear that they are creators of cards. And since cards replaced servants in ritual, then is safe to assume that Ainsworths replaced Einzberns role in creation if ritual.
Who remains are Sakatsukis, which don't really seem as Magus family, but rather as normal family gifted with natural miracle. Therefore I would personally guess that they aren't Founding Family, but rather simple family which is used by Founding Families, as it was stated that this Grail War ritual was created to utilize natural Miracle and not using own artificial one.
Natural miracle? Miyu confirmed to be a mixed-blood psychic.

Good god, I just realized how depressing Miyu!Shirou's life is.

First his friend turns out to be evil, then his imouto gets kidnapped, then he gets skewered, and then he wastes 3 months trying to get to her. Then Sakura comes along and confronts him about the HGW, crushing whatever good things he has left in life. But she admits that she loves her senpai and is willing to run off with him, and he actually contemplates the choice and likes it enough to imagine being married with her (see the ring?). But against his internal turmoil, he chooses to save his imouto instead, but then Shinji comes along to fuck things up. Sometime later he saves his imouto and gets captured, but then she gets kidnapped again three months later.
Surprisingly, I'm liking this Shirou a lot. If I had to say, he's currently my favorite Shirou out of all the non-VN Fate works and outright my favorite character in Prillya. Hiroyama's doing a really good job with him, especially when portraying his internal thoughts and conflicts.
