Sigma's arrow was created with the intent to shoot a "god" down from the sky. It bypassed Ishtar's protections and struck her left arm. The left arm fell out of the sky and the gallû take hold of it so the shadow can fill it with death.
That left arm being filled with death was enough to start dragging her soul down to the underworld.

Orion's shadow makes some jibes at the shot, saying its cheating if you use a bow made for shooting down a god since its reversing cause and effect.

Depending on the characteristics of the Grail, the Heroic Spirits that can be called upon and cannot be called upon will change. Because Heroic Spirit with deep karmic ties are usually summoned, its difficult to summon Servants with ties to the Orient or say North America with a Grail made by European Magi like the Fuyuki Grail. The characteristics of the Grail take precedent over the leylines.
