... I think you misunderstood. I know the whole spewing blood is from the TB, it's part of her Weak Constitution skill thing. But if the Shinsengumi uniform raises all her stats, including endurance, it stands to reason it would lessen the strain on her from the TB while performing a move that breaks causality. So while she might not 'need' to invoke her uniform to use the move, it might help her do it without completely keeling over afterwards.
It's also possible that maybe she needed the boost in stats to bring down Nagayoshi, cause he took a spear to the skull before from a guy called 'God Spear' and still didn't frickin' die.
Since everyone has been calling Okita's Noble Phantasm a Tsubame Gaeshi rip-off, I should note that ironically Okita actually has more real-life mythological claim to the Multi-Dimensional Refraction Phenomenon concept than Kojirou.

Nah, in popular folk lore and legend, Tsubame Gaeshi is like multiple strikes at god-like speed so one can romanticize Musashi even more, whereas in reality it is believed to be a changing-direction attack. Just like, in popular folk lore and legend, Sandanzuki has the embellished multiple strikes at god-like speed to romanticize the sad fujoshi dream of this genius sword boy that died too young, but in reality was probably just three quick strikes.
Watch as Trust and Betrayal even shows how potentially slow it could be!

Indeed, of course it's an embellishment, but the Fate universe runs on power of beliefs and legends. My point was that Okita actually had the three simultaneous strike as part of his legend. In contrast, Tsubame Gaeshi has godlike-speed, but it's very name implies it's not about being simultaneous, but rather lightning fast turning of the blade to slash in a reversed direction ("swift cuts downward and then immediately upwards. Hence, the "Turning Swallow Cut" has been reconstructed as a technique involving striking downward from above and then instantly striking again in an upward motion from below. The strike's second phase could be from below toward the rear and then upward at an angle, like an eagle climbing again after swooping down on its prey. Sasaki created this technique around 1605.") to follow the target.
So Kojirou's technique sounds potentially faster, but at least in real life the legend of the technique is about turning/reversing the blade, and thus has a name that translates to "Turning Swallow Cut" or "Swallow Reversal/Return". Hence it's by definition not supposed to be simultaneous.
