That's correct: Kokutou is the surname. I was using Eastern name order, last-first.

I've always thought Mana was a contradictory existence. Her origin must be spectacular in order to escape the void. I mean a void is supposed to nothing, barren, empty, suck everything in and let nothing out. But she seems like a pretty normal girl most of the time.
