I don't think that doing a main interlude of an event which did not have a welfare Servant would be very well received when you can still bring back so many old welfares.

Still wishing for Onigashima MI, hoping to get my own Ridertoki.
Did Prison Tower have welfare?
No, and it’s already in the RP shop
Doylist reason Watsonian reason Artoria Otome game turned eroge and they didn't want to change main love interest She pretended to be a man because the people wouldn't accept a female king Nero Build up intrigue in Fate/EXTRA's marketing with a completely irrelevant red herring None, but Nero claims she's dressed like a man and that's why people got confused Altera ??? I only played one route of Extella Okita Keikenchi parody, also the real one was bishie enough to be mistaken for a woman None Mordred Astolfo already filled Apocrypha's man who looks like a girl quota Arthuria's clone Musashi To make her a separate character with a different backstory from man Musashi Alternate version from a pruned world Pollux To make that twincest straight, probably None Ibuki They made Shuten a girl first None Enkidu (genderless) Beep boop divine robot Beep boop divine robot Jaguar Man (pseudo) Pseudo Pseudo Nezha People have been genderbending him for millennia already Buddha made his second body a girl without asking his input Kagetora Historically speculated to be woman in disguise None Gareth Unknown None Quirinus (genderless) Transcending gender is a sign of power Transcending gender is a sign of power Vritra Unknown Doesn't have a normal humanoid form and is historically known to love hot women, so she decided her human form should be a hot woman Nobunaga Keikenchi parody, also the real one was often mistaken for a woman in his teenage days None Ushiwakamaru Takeuchi wanted to "fix the gender ratio" and Yoshitsune was already genderbent by video games a few times before None Drake Unknown Was manlier than the male pirates, and might be Queen Elizabeth Quetzalcoatl Unknown Aztec gods are known to act by possessing humans, so they went with a case of a male god possessing a female body Sima Yi (pseudo) Pseudo Pseudo Sanzang Have you seen any non-genderbent Japanese version of Sanzang in the past 50 years? None Da Vinci Unknown Wanted to be the Mona Lisa Shuten Unknown None Kama (pseudo) Pseudo Pseudo
Kiiichi Unknown (Sakurai was just trying to make a character that appeals to all of her kinks at the same time, maybe?) Tengus are shapeshifters and she has a male form she's not showing us Frankenstein Unknown (Frankenstein's Bride references, most likely) Mary Shelley wrote the book different from the real magecraft experiment it's based on, probably for concealment of Mystery purposes Ibaraki Shuten's wife in some versions None Raikou Takeuchi felt like it Lived concealing her identity because of the whole Ushi Gozen deal Bunyan Riyo wanted to make all his 7 Servants be women (with 6 being genderbent) for the funnies None Qin (genderless) Transcending gender is a sign of power and also a Kars reference Transcending gender is a sign of power and also Elmo 2 Adventures is implying Zhenrens are all genderless Cupid (pseudo) Pseudo Pseudo Kagekiyo They made Ushiwaka a girl first Hivemind of everyone who wished for Yoritomo's death, including Yoshitsune Ganesha (pseudo) Pseudo Pseudo Douman (genderless) The Watsonian reason probably came first on this one Ate three gods and not all of them were male Gogh (kinda?) Amphibian was hired to make a Nemo event and Nemo is a guy with identity issues over being a fusion, so his OC needed to be someone with identity issues over being a fusion to foil him Vulthoom needed to fuse van Gogh with someone with a story about never dying
35 genderbends in FGO so far, unless Herod missed someone.

Forgot:
Jing Ke - no reason
Jack - he could've been anyone or anything and an amalgamation of stripper kids is only one of those
Danzou - ???
Hokusai - the name of the servant is Katsushika Hokusai but the actual servant we have is Ōi so guess this one is kinda whatever
Also, the reasoning for Altera is that she's a big alien weapon of mass destruction (Sefar) that was sent to Earth to do alien WMD stuff, but she was beaten there and from the giant corpse they excavated the "brain", which is what Altera is, and raised her as a human. So there is an explanation there, but there's no explanation for why she somehow because of that replaces actual Attila in history.
Jing Ke is because the Japanese read her name as Keika and simply can't convince themselves that Keika can be a man's name.
Spoiler:
More than I expected. Thanks for the list.

Considering all the Mother Harlot hype, I still wonder why Nero being female wasn’t Innocent Monster influence from the Whore of Babylon and the associated myths. Extra started Innocent Monsters with Vlad, and the skill causing a genderbend wouldn’t be too crazy in the grand scheme of things
waifu can't really be a man because then you're gay or something
And yet the same game gives waifu EMIYA.
That reminds me, I read somewhere that Nasu mentioned the three playable Servants in OG Extra that are now return in Extella can be described as heroines who have grown mentally after spending their time with Hakuno during Extra. It gave me a chuckle too.
Thanks
Still waiting for man Musashi zzzz
Something
funny thing about Sanzang in Japan. they did live action series of the Journey to the West and they used female actress for Sanzang.
though Sanzang is a male in story.
Maybe they thought it'd be too yaoi
