It's hard to say for sure because we don't really have good full body shots of Kay's novel design. There's just this and a silhouette from the movie (identified by the shoulder plates). But based on manga Kay's more worn and chain looking outfit I don't think it's the same as novel Kay's plate.

We’ll see later but I think the reasonable assumption for now at least is underneath all of the extra stuff they gave him his novel design is still mostly there.

Also, while we're on the subject. I've always wondered and now seeing that Kay's right eye is shadowed in the manga, is that combined with his novel depiction seemingly lacking a pupil in the same eye meant to imply that he only has one working eye?

The neck part is the same, or at least the throat part. Or close enough. The actual neck+shoulders part seems to be gone though.

That or he too has a Mystic Eye. Wouldn't be the first time they added it gratuitously to a character.

Certainly possible. The reason I'm leaning towards disability would be because it would pair him up thematically with his eternal bromance with Bedivere. I swear I've seen that in a shonen manga or anime before where two characters have some kind of impediment and they have to cover for each other's blind side in battle.

Honestly, with the exception of Mordred, I wouldn't be surprised if the rest of Arthur's version of the KoRT just look identical inexplicably to avoid wasting time on character redesigns for people who are only likely to be seen in Arthur's flashbacks and whatnot.

It's not like Arash, Bryn, Paracelsus, etc. look any different between Prototype and other settings. Gil, Merlin, Arthur, and Mordred seem to be the exceptions not the rule.

I'm pretty sure the only reason Proto Mordred is different is that he shares the backstory of being a clone of Arthur.

It looks a lot like Add’s designs on him so I just assume that it might be a quirk with Add’s projection

Might look the same beneath the helmet. Remember, in the novels, Kay was "hazy" to normal observers like he would vanish into thin air, and Gray only gets a good look at him at the very end when he sacrifices himself and deploy his Virtual NP against the Bird of Hermes.

For the first time, only for a moment, I saw his face. That was probably because of the large concentration of Magical Energy that he had gathered for the simulation of a Noble Phantasm.

It was a face that looked slightly troubled, yet carried a smile that looked like it belonged to someone with a terrible personality.

Because he's not a Servant here, he's an AI copy made with Atlas tech projected outside of Add. Just using his NP once as a Virtual Noble Phantasm ("virtual" as in the same word use for Lord Chaldeas before it became Lord Camelot) is enough to burn out his Spirit Origin and kill him.
