Oh yeah, I remember now, reading about how Ayako was somehow involved in the RN version.
This sequence takes place sometime after the incident at Ryuudou temple with Medea, right? I imagine the Medea/Ryuudou scene isn't so far off from the ending of the film, and if that's the case, would there be anytime for the first HF film to indicate how ecchi the trilogy is willing to be? (I know, that's an HF Film thread question, but still.)

Blood sucking would look stupid in the movie, I would imagine. They don't need to literally animate them having sex, I mean in Prisma Illya they went with French kissing which would work well for the Rin x Shirou scene.. Just show them hornier than usual and add in disturbing red static effects as they go even more physical. HF is all about robbing away the audience's understanding of the established Fate/UBW tone. Sakura sucking blood doesn't even show how batshit crazy she is being obsessed with pleasure with her Senpai. It would destroy an integral part of Sakura's character.

But the bloodsucking was really creepy, so it fit with HF mature theme. It was also homoerotic so that's always a plus.

When I first read HF 3 years ago (14 year old me didn't dare to read it and I put it on the backbench until I was 20) l didn't feel that Sakura was creepy. The text and plot made it clear that she was batshit crazy, which IMO bloodsucking doesn't fit what the plot wanted us to feel.
I mean , in Realta Nua, Shirou keeps on saying he loves her, and Sakura says she's not worthy of Shirou's love because all she wants is to suck his blood. Shirou says it's okay and she's beautiful. Then Sakura kisses Shirou's cheek. It just feels wrong and it doesn't emphasize Sakura's obsession with Senpai, both physically and mentally. Now compare this with the original scene. Sakura straight up devours Shirou with lust, making them become animals. It reinforces their unhealthy and destructive relationship 10 times better than what the RN scene conveys. Its mutual insanity due to physical intimacy and magical energy drainage.
Plus, why would Shirou be turned on due to bloodsucking? It just feels out of character for him to be suddenly be into bloodsucking fetish. It would feel way more natural and relatable for high school students being obsessed with sex and pleasure.

Explicit sex scene or not, the combination of RinXAyako and blood-sucking reads more erotically than one may expect for an 'all ages' version.
If the film is going to lean harder on Realta Nua than the original for the intimate scenes with Sakura, they may as well do that here, too. Not only is it kind of creepy, it also seems to provide a more specific kind of foreshadowing than "merely sex" would provide to the aforementioned Sakura scenes.

You cannot go wrong with Tohsaka H-scenes.
Finally Tohsaka sauce is so damn gooddddd!
It wasn't their relationship being destructive or unhealthy so much as it was clearly foreshadowing Angra Mainyu in the beginning, because usually, you wouldn't get this urge (which the internal monologue highlighted) to literally rip apart the person you are being intimate with.
Plus this part of it was absent for the next two scenes, it only showed up in the first one. The last one was sort of sweet with the awkwardness, but then the Nightmare interlude afterwards was like whoa.
It had that jarring back and forth that was the thing.
Then you have the scene in Hollow which was pretty much the chillest, most relaxed casual shagging of the lot, so Sakura gets the whole gamut.
Also I'm intrigued, but not surprised to see the Rider dream sequence here. The guy doing it does make ero doujins.
