Edition-independant, you call for a check anytime there's a chance of failure AND that failure can actually lead to something interesting. Falling off a knotted rope can alert a nearby guard during your caper, failing to recall the name of the neighbouring country might tip someone off that you're an outsider, etc. They're something you SHOULD be able to do easily, so the DC is low, but there is a check regardless.
I'm not even gonna address the "uhh but a competent DM tho!" slight.

To explain how this works...
Diablo 1 (the video game) is a game where it's quite easy to get randomly dogpiled by a swarm of monsters and killed out of nowhere, or to be struck down by massive damage from a single enemy. There's a fear to it. I wanted to preserve this sense of danger and horror but without making it so you have to churn through characters once every ten minutes. Everyone put effort into their characters, so I want there to be some sense of investment.
At the same time, Diablo is a story filled with heroes fighting evil that often ends with the heroes becoming twisted by their experiences.
As a result, there is a mechanic called Corruption, which is sort of like Humanity from Dark Souls. When you die, you get a rank of Corruption and then can continue on, but with some madness, trauma, or evil tainting your heart. The more corruption you get, the more twisted you become, until finally your character no longer able to be the hero that they once were. (You can get corruption from non-lethal sources as well, and there are ways to die that just kill you outright instead of giving you corruption.)
So rather than just death, it represents a negative character arc, a downfall. At that point, the character becomes an NPC.
To bring it back to TM, you could say that Dark Sakura has her corruption maxed out.

I really have trouble seeing the DS in it.
Basically, you die. You might spontaneously rez, but you come back less than you were before; less sane, less human.
In DS, you die and lose your human form, but you can reclaim it. Humanity itself is arguably dangerous; it can straight up attack you in the right conditions and is implied to do horrible things to the Firekeepers. You ho nuts in DS and Hollow because you literally can't die...
