Why are we assuming she is smug and acts victorious when she only went to him after getting Saber again?
I mean, if you need to bring Saber to fight Kirei when he's "old", why would she underestimate him when she's alone fighting a more fit version?

...Actually, I think I'm going to reverse my stance: until she acquires more information on her enemies, Medea tends to underestimate them. She underestimated Archer and was outmaneuvered by him in her own Temple, she underestimated Saber's Magic Resistance and Kuzuki didn't die because of that only thanks to his fighting technique, and she underestimated Rin and got beat up; her best plans were only successful because she already had a good estimate of the strength of her targets. I don't know if Zero!Kotomine would be capable of pulling off the same trick Rin did (I do think she had a lot of "protagonist power" in that scene), but if it depended on Medea underestimating him, I think he could do it...
...But would he? Zero!Kotomine doesn't strike me as the type of fighter who relies on gambits. In his fight against Maiya and Irisviel, and later against Kiritsugu, he was a very in-your-face melee fighter.
Wasn't Saber basically paralyzed anyway? I doubt Medea needed her help or anything.
