Not really? I mean even without the rape, he basically tried to beat her head in and drag her into the room upstairs while she was basically an invalid.
Or the fact he drew blood earlier in the route (Shirou finds it when he came back after she got kidnapped from his house before the school scene).

As for h-scenes, even as a Sakura fan they tried to entice the creep factor too much with Shirou being homicidal in internal monologue sometimes. The last one I think being the only one that wasn't shy of semi-psychotic with a cute hug and sweet adorkableness between them.
Rider using Rin's likeness also had the hotness ruined by being just a little too predatory. The threesome in Fate was good for Rin just letting it all hang out, to the point I can't even blame Shirou firing off early.
UBW had a realistic start and then for no reason just went off the rails for the sake of it, so if Nasu and Takeuchi were tag teaming that, they both fail for not seeing how they destroyed the atmosphere.

What I meant is that Sakura looks psychotic and unbalanced, even if she is sympathetic. There is always that element of horror throughout HF that boils over when she kills Shinji and becomes Dark Sakura.

Personally, I am sympathetic towards Sakura and think Shinji had it coming, but it is still kind of disturbing when Sakura finally crosses that line.

Exactly, just like how most of Sakura's H-scenes, and even the domestic scenes, feel somewhat unsettling at least and fucked up at most. Sakura is always meant to be somewhat disturbing, even if you like her and sympathize with her.

Yeah, the disturbing part is afterwards where she thinks about how killing him didn't make her feel anything, how she should have done it earlier, and how it was fun and she can't wait to sadistically murder everyone else and her sister in particular (and it should be, since at that point she was having a full-blown mental breakdown).

Also, keep in mind that the only reason she actually does this is not to protect herself... it's because she views herself as belonging to Shirou now. In a sense, this is also part of why she puts up with Shinji raping her in the first place; she views herself as having stolen his status and identity as the Matou's successor. Not because he's more powerful or anything. The scene makes this abundantly clear. Even half-trained and near-invalid, she's still able to just casually slaughter him. She's pitying him. It is all kinds of messed up, and just one part in a series of events that makes it clear Sakura is insane.
