I highly doubt he'd sleep with his sister.
He just has a massive case of survivors guilt, since he got away unscratched and she lost her leg's, and the use of her eyes.
Then again, they are Britannian royalty and incest is kinda traditional for such families. Besides, being brother and sister didn't stop his feelings for Euphie and it's not like you can say he didn't view her as a sister since she was his childhood friend and thus the Westermarck effect or whatever should also be in action but clearly isn't.
Besides, Nunnaly has been implied to have less than pure thoughts about him as well.
