Nick, if you really want to rec a fic, read over what everyone else has shared. They rec stories they love, find great, and want others to know of their greatness. Not finding obscure niche titles for the sake of it. (It can be both, but I advice against that for now)
I love my niche RPGs as much as the next guy (Luminous Arc, Atelier series, Time and Eternity, Beyond Good & Evil, ect), but they don't really apply to OTHER people's tastes. Niche means a small target.
You want to look for the Marios or Zeldas; fics that feel fresh despite being similar. Arashi writes smut but damn it's the good smut. With me so far?
What you DON'T want to look for are the Final Fantasies; fics that LOOK and READ good but are actually structurally bad once you read into the common fanon stuff. Like "In Flight".

I was surprised how much I enjoyed this story while reading it. Other than Dezo Penguin's Rin and Rider entry a few years back and IRun's story from last year, this has been the most enjoyable fanfic I've read while judging the previous BL contests. This story comes off firing on all cylinders with Nero making short work of an underground gambling ring (with a fucking tiger shark!) and Ciel making her . . . desires known.
There's some great language and writing here. However this isn't for the faint of heart. After all, this is porn with Nero for christsakes.

If the protagonist is somewhat insane, and thinking about killing his lover with every thrust he makes, erotic horror?
A negative vote to Chaos Theory.
It started well, that much is undeniable. But the longer the story goes on (and the longer Moczo takes to update between chapters), the more the flaws become apparent. Caster Alter is a large one; her EVIIIIILLLLLLLLLLLLLLLLLLLLLLL histrionics and antics quickly overstay their welcome, as well as her awkward yuri, and due to her status as Moczo's favorite character has survived many situations she shouldn't have. Another is Saber's partial blackening, a hackneyed plot development at best that is a pale representation of HF events and completely ejected me from the story as he unceremoniously disposed of Berserker. A third issue is Shirou's lack of agency. While HF had thematic reasons for delaying his development of fighting ability, those reasons have not been expressed in Chaos Theory, leaving him a glorified battery whose primary purpose is being the punchline to Rin and Saber's increasingly unfunny disdain.
Chaos Theory started well. However, its myriad flaws me ejected from the story entirely, and I cannot recommend the disappointing experience of seeing a good fic go bad.

Then again, I'm not up to date, so maybe it got worse since I stopped reading it. It sounds like Moczo dug himself a hole and started magnifying the simplest aspects of certain characters.
