Caesar’s favorite weapon, the wounds inflicted by this blade cannot be healed. Furthermore, the target’s health starts to quickly deteriorate from the moment he is injured by this Noble Phantasm, causing his death not long afterwards.
Despite its ominous ability, this is no magic sword・cursed blade, but a genuine holy sword. A divine treasure forged by the God of Craft Vulcan. Later, the goddess Venus gifted it to her son Aeneas – founding-hero of the Roman civilization. Lost during the early ages of the Roman Republic, it was gifted to Caesar during his youth – a clear sign that the gods had great expectations of him as a harbinger of a new age for Rome.
Those that oppose this sword and its wielder are recognized as “an enemy of the gods”, and thus will have their lives forfeit by means of heavenly mandate. What prevents the healing of injury and degrades the health of the body is no poison or disease, but a divine curse that enforces the demise of the target.

Saber’s privilege as someone favored by the gods turned into a Noble Phantasm. In life, Caesar was ensured the accomplishment of all his endeavors for as long as he fulfilled his duties as an attendant of heavens. Even now, after his sublimation into a Heroic Spirit, this promise is still in effect.
Activation requires the performance of an extensive religious ritual, in which the appropriate sacrifices must be offered to honor the Roman gods. Once this requirement has been fulfilled, the power of heavenly providence will descend onto the world and bestow Caesar with matchless divine protection. Even if only for a limited amount of time, he will have assured success in all rolls, while his enemies will continuously suffer failure on their own rolls when attempting to directly interfere with him. In addition, Caesar’s allies will also enjoy the benefits of this Noble Phantasm during group battles – although in a more limited fashion.
Still, due to the very nature of this Noble Phantasm, it displays little effect against enemies that have Divinity at a high rank.
If one went only with its perceivable effects, this should be classified as an Anti-Unit, Anti-Army Noble Phantasm. However, since even the natural order of the planet is being interfered with by means of reversal of causality・rewriting of events, it instead falls into the Anti-World category.

I wouldn't mind Caesar having an EX NP, but I'm not sure why you think something like that classifies as Anti World or EX just for reversing causality when Gae Bolg is only B rank and is much more dramatic. Also, it's pretty weak. Not all abilities use saving throws and not all those that do are save or die type effects like Gae Bolg.
Semiramis's Hanging Gardens of Babylon revise all attacks to be in her favor as a minor side-effect.
That way of thinking is too naive. When I say "all rolls" I mean ALL ROLLS, not just saving throws. While this NP is active, all his attack rolls, evasion rolls, parameters checks, skill checks and likes will have automatic success. Meanwhile, enemies will suffer automatic failures in those same tests when engaging against him. Just think along the lines that "Caesar will be successful in whatever he does, while opponents will always fail in their attempts to hinder him".
Basically, I gave him an even more overpowered (albeit time-limited) version of that cheat Skill that Meltlilith from Fate/EXTRA CCC has.

Sorry if I'm being repetitive here, but I simply can't stress enough how broken this NP is supposed to be. It employs large-scale amendments of the natural laws in order to always bring about the most convenient developments for Caesar. If this Servant were to become the main character in someone's story one day, VVV would give said author the perfect excuse to use a deus ex machina-like plot device in order to guide the story towards whatever outcome he wants.
