It is Touko, but it is the Touko in the Tsukihime/Fate Universe. If you want to link a deed to this version of Touko, it is the one who crafted Shirou's puppet body post Heaven's Feel.
Tsukihime and Fate share the same universe, while KnK stands alone and runs parallel to it.
Why can't Touko meet Arcueid again? I still think this separate universes thing is a bit silly, and was probably referring to narrative stuff rather than dimensional metaphysics.

It's all from something on one of those plus discs, I think. The gist being that main characters aren't going to be bumping into each other, but side characters can do whatever they want.

The actual entry I posted was from the dictionary of KnK. It is alluding to Tsukihime (with the foreigner vampire and nun).
The inference is that, the two universe are independent of each other, with the cafe being the single point joining the two universes.
First, Arcueid said she and Touko from "KnK" cannot meet because it would create inconsistencies (same with Ryougi and Tohno). And second, she said that Tohno meeting someone like Asagami Fujino would not be a problem. Also, take a look at Ryougi's win quote against Arc in MBAA.
